Our Mission

UHPG is committed to transforming the health and well-being of immigrant, refugee, aging, and underserved communities. Through culturally responsive healthcare, education, mental wellness services, workforce development, and social support, we meet people where they are. By building innovative community partnerships, we create equitable pathways to care, opportunity, dignity, and healthier futures for all.

In Colorado, UHPG envisions a future where every family—regardless of background, income, age, or circumstance—has the resources and support needed to achieve lifelong wellness. As a leading community-driven healthcare organization, we work to eliminate health disparities, strengthen access to care, and empower communities to thrive with dignity, confidence, and hope.

Our 5-Year Strategic Objectives & Targets (2026-2031)

  • By 2028, expand outreach programs into 20 underserved communities in Colorado; by 2031, scale to at least 36 communities regionally.

  • By 2029, provide preventive health screenings to 3,500 individuals annually; increase to 6,000 annually by 2031.

  • By 2029, conduct 30 health education workshops per year in Colorado; expand to 50 annually by 2031 with broader topic coverage.

  • By 2030, establish at least 8 formal partnerships with healthcare providers, nonprofits, state of Colorado agencies and Federal agencies; grow to 12 active partnerships by 2031.

  • By 2029, recruit and maintain 100 active healthcare professionals and volunteers; increase to 135 by 2031.

  • By 2029, launch 12 mobile outreach clinics serving at least 4 counties; expand service coverage to 16 counties by 2031.

  • By 2031, increase participation in preventive care services among target populations by 30 percent, measured through program data and follow-up engagement.

  • By 2029, implement a healthcare navigation program assisting at least 2500 individuals annually; expand to 3500 individuals per year by 2031.

  • By 2029, integrate telehealth services reaching at least 3000 patients annually; scale to 4500 patients annually by 2031.

  • By 2029, secure at least 5 sustainable funding sources (grants or partnerships); increase to 8 funding streams supporting long-term growth by 2031.
