Who We Work With

In Colorado, UHPG works with organizations and professionals who share a commitment to improving community health, expanding access to care, and promoting prevention.

Our Partners, Donors and Supporters.

Healthcare Providers and Clinical Partners

Healthcare providers and clinical partners contribute medical expertise, preventive services, screenings, health education, referrals, and professional guidance. Their involvement helps UHPG connect community members with trusted care, identify health concerns early, and promote informed decisions. These partners strengthen service quality, continuity of care, and the overall impact of UHPG programs.

Hospitals, Clinics, and Community Health Centers

Hospitals, clinics, and community health centers are essential partners in expanding access to quality healthcare. They provide clinical referrals, preventive screenings, vaccinations, specialty services, and follow-up care. Through collaboration, UHPG helps individuals navigate available services, schedule appointments, and receive timely, coordinated support that improves health outcomes for families across community.

Public-Health Agencies and Government Programs

Public-health agencies and government programs help UHPG advance prevention, health education, vaccinations, screenings, emergency preparedness, and access to vital resources. These partnerships connect communities with trusted information, public benefits, and coordinated services. By working together, UHPG can address health disparities, strengthen outreach, and support outcomes for individuals and families.

Nonprofit, Faith-Based, and Community Organizations

Nonprofit, faith-based, and community organizations help UHPG reach people in familiar, trusted settings. They support outreach events, health education, volunteer engagement, referrals, and culturally responsive services. Their community relationships help identify local needs, reduce stigma, and connect individuals and families to practical health resources, support, and opportunities for improved wellbeing.

Corporate Sponsors, Local Businesses, and Foundations

Corporate sponsors, local businesses, and foundations provide financial resources, in-kind donations, event support, professional services, and community visibility. Their investments help UHPG purchase supplies, expand outreach, support educational programs, and reach underserved populations. By investing in community health, these supporters demonstrate social responsibility while helping create stronger, healthier communities together.

Individual Donors, Volunteers, and Community Advocates

Individual donors, volunteers, and community advocates are the heart of UHPG’s mission. They give financial support, time, professional expertise, encouragement, and valuable connections. Their involvement helps deliver events, share trusted information, assist families, and build awareness. Every contribution, regardless of size, helps UHPG extend care, promote prevention, and strengthen wellbeing.

Call For Grants, Financial and In-kind Donations

Grants Needed

UHPG seeks grant funding to strengthen and expand its comprehensive community health programs, including Community Health & Wellness, Community Health Outreach, Health Education & Prevention, Healthy Aging Services, Mental Health Awareness and Support, Healthcare Navigation & Resource Assistance, and Community Wellness Campaigns.

Funding is needed to support preventive health screenings for blood pressure, diabetes, cholesterol, and other health risks; vaccination clinics; women’s and men’s health events; nutrition education; chronic-disease prevention; mental-health awareness sessions; healthy-aging resources; and community-based wellness campaigns focused on heart health, diabetes prevention, immunizations, and healthy living.

Grant support will also enable UHPG to provide outreach staff, trained volunteers, healthcare professionals, screening equipment, medical supplies, educational materials, interpretation services, transportation assistance, meeting space, technology, and communications support. Funding will strengthen referral networks and help individuals access primary care, specialty care, behavioral-health services, insurance enrollment information, prescription assistance, and other essential community resources.

Through grant investment, UHPG can reduce barriers to care, reach underserved communities, promote early intervention, and build healthier, stronger, and more informed individuals and families.
