Our Core Programs
UHPG delivers a range of community-centered health programs designed to improve access, promote prevention, and support lifelong wellbeing. Its core programs include Community Health & Wellness, Community Health Outreach, Health Education & Prevention, Healthy Aging Services, Mental Health Awareness and Support, Healthcare Navigation and Resource Assistance, plus Community Wellness Campaigns.
Community Health Outreach
UHPG’s Community Health Outreach program brings trusted health information, preventive services, and referral support directly to the communities that need them most. Through partnerships with healthcare professionals, local organizations, faith leaders, and volunteers, the program reaches individuals who may face barriers related to cost, language, transportation, or limited access to care. Outreach activities provide education on disease prevention, healthy living, mental wellness, maternal health, and available community resources. By meeting people in familiar, welcoming settings, UHPG promotes early action, strengthens health literacy, and encourages ongoing connection to healthcare services. The program helps build healthier, informed, and more resilient communities together.

Health Education & Prevention
UHPG’s Health Education & Prevention program empowers community members with the knowledge and practical tools needed to make informed health decisions. Through workshops, presentations, resource materials, and community conversations, the program addresses preventive care, chronic disease management, healthy lifestyles, mental wellness, maternal health, and early warning signs of illness. Participants learn the importance of regular checkups, vaccinations, physical activity, balanced nutrition, and timely medical attention. Education is delivered in clear, culturally responsive, and accessible ways that respect diverse backgrounds and experiences. By promoting awareness before health challenges become emergencies, UHPG helps individuals, families, and communities build healthier habits and improve long-term wellbeing.
Healthy Aging Services
UHPG’s Healthy Aging Services program supports older adults in maintaining independence, dignity, and quality of life as they age. The program provides health education, wellness resources, preventive screenings, referral support, and opportunities for social connection. Participants receive guidance on managing chronic conditions, staying physically active, maintaining balanced nutrition, taking medications safely, and recognizing changes that may require medical attention. UHPG also connects seniors and caregivers with trusted community services that address transportation, housing, caregiving, mental wellness, and access to healthcare. By promoting active aging and reducing isolation, the program helps older adults remain informed, engaged, supported, and connected to their communities.

Mental Health Awareness and Support
UHPG’s Mental Health Awareness and Support program promotes emotional wellbeing by helping community members understand stress, depression, anxiety, trauma, and other mental-health concerns. Through educational sessions, peer conversations, resource sharing, and referrals, UHPG creates safe, welcoming spaces for people to learn, ask questions, and seek help without shame. For example, a community session may help parents recognize signs of emotional distress in children, understand when additional support is needed, and identify trusted places to obtain care. By addressing misinformation and stigma, the program encourages early intervention, strengthens family support, and connects individuals to appropriate mental-health services and community resources.

Healthcare Navigation & Resource Assistance
UHPG’s Healthcare Navigation & Resource Assistance program helps individuals and families understand, access, and use available healthcare and community-support services. Navigators provide clear guidance on finding primary and specialty care, scheduling appointments, preparing for medical visits, understanding referrals, and locating low-cost or no-cost services. The program also connects participants with insurance enrollment information, prescription assistance, transportation options, food resources, and social services that support overall wellbeing. By simplifying complex healthcare systems and addressing practical barriers, UHPG enables community members to make informed choices, follow through with needed care, and obtain timely support. This service promotes equitable access and improved outcomes.
Community Wellness Campaigns
UHPG’s Community Wellness Campaigns raise public awareness and encourage healthier choices across the communities it serves. Through coordinated outreach, the organization shares practical information on heart health, diabetes prevention, vaccinations, physical activity, nutrition, and other priority wellness topics. Campaigns may include flyers, social-media posts, educational materials, neighborhood outreach, and short presentations at community events, workplaces, schools, or faith-based gatherings. For example, a heart-health campaign can help residents understand risk factors, recognize warning signs, and identify simple steps to protect their wellbeing. By delivering culturally responsive messages through community channels, UHPG promotes prevention, inspires action, and supports healthier living for all.
